Verifying the system, not signing the permit

A consultant who signs permits quietly takes on the issuing role. The assurance position is different: sample permits in the field, compare them against what is actually happening, and record the gap. That keeps the duty where the contract and the law put it while still catching failures.

Before issue

The checks that prevent most permit failures happen before anyone starts.

  • The method statement and risk assessment match the activity, location and crew named on the permit.
  • Competence for the specific task — the operator, the supervisor, the fire watch, the standby person.
  • Isolation and lock-off arranged and proven, with a register entry that names who holds the lock.
  • Gas testing, ventilation or atmospheric monitoring where the activity requires it.
  • Interfaces cleared: nothing else is planned in the same space at the same time.

During the work

A mid-shift check is worth more than a morning file review. Is the permit displayed at the point of work? Are the conditions on it still true? Did the crew change? Is the fire watch present for hot work, and are extinguishers and screens where the permit says?

Hand-back and close-out

Closing a permit means the area is left safe, isolations are removed under control, the fire watch has run its full period, and the record is filed against the activity. Where a permit is extended or reissued, the extension needs the same checks as issue rather than a signature on the original.
