The second incident happens during the recovery
An excavator drops into soft ground, a telehandler tips, a truck sinks on fill. The damage is usually survivable — what turns it into a fatality, a stop-work notice or an unanswerable authority question is the improvised pull that follows. This module controls the whole sequence: emergency stabilisation, engineer-led survey, approved recovery plan, notification deadlines, return-to-service release and CAPA close-out.

Stop / do not move
The Incident Commander issues a written STOP / DO NOT MOVE instruction immediately. No person may enter the crush zone, go beneath the plant, enter the cab, climb the machine, operate controls, attach improvised slings or chains, use another machine to pull, or raise a body/boom until the recovery plan is approved and the recovery supervisor releases the task.

Recovery sequence
7 stages from stabilisation to lessons learned
No recovery method may be selected before the engineer-led survey is documented. The sequence is enforced in the checklist, so a stage cannot be quietly skipped under production pressure.
Stage 0 — Emergency stabilisation
Account for driver, passengers, banksman and nearby workers (999 Police, 998 Ambulance, 997 Civil Defence as required). Stop all work and vehicle movement, barrier beyond the credible roll / boom / rigging / recovery-line hazard zone, identify fuel, hydraulic, battery, load, overhead and fire hazards from a safe location, and preserve plant position, ground marks, CCTV, telematics, permits and photographs.
Stage 1 — Engineer-led technical survey
No recovery method may be selected until the survey documents the plant (mass, load, attachments, damage, ROPS/FOPS, manufacturer recovery instructions), stability and secondary-roll path, ground condition (fill, saturation, voids, utilities, edge distance), recovery access and the supporting documentation. If a safe recovery-equipment position and bearing condition cannot be established, the recovery does not proceed.
Stage 2 — Written recovery engineering plan
Task-specific method statement, risk assessment and lift/recovery plan stating confirmed mass and centre of gravity, the method limited to manufacturer-approved points, rated capacity at the actual configuration, design-verified loads at every critical stage, geotechnical verification of each machine position, exclusion and recoil zones, one command structure with hold points, and a contingency for ground movement, rotation, rigging damage and fuel release.
Stage 3 — Soft-ground preparation
Engineer-approved bearing and settlement basis for each recovery machine; designed platforms, mats or bridging only — never stacked timber, rubble or mixed improvised support; water source identified and managed; engineered stand-off from excavation edges, retaining systems, buried structures and suspected voids; proof loading or staged monitoring completed before equipment enters position.
Stage 4 — Pre-recovery permit and briefing
Documented briefing verifying authority / client / insurer direction, approved plan and permits on site, named recovery team and rescue support, equipment certificates and pre-use checks, manufacturer-matched connection points, solid barriers and sentries, and a reassessment of ground, weather, lighting, wind and communications immediately before work.
Stage 5 — Controlled righting and execution
Single command, gradual slack take-up with a pause at first tension, one engineered stage at a time with supervisor confirmation at each hold point, remote observation positions only, and an immediate stop for cracking, sinkage, rocking, unexpected rotation, rigging damage, load shift, fluid release, lost communication, weather change or unauthorised entry.
Stage 6 — Quarantine and return to service
Righting does not release the plant. It stays quarantined until a competent plant engineer, owner or manufacturer issues a written assessment of ROPS/FOPS/cab, chassis, wheels/tracks/axles, steering, brakes, suspension, body/boom/hoist, hydraulics, electrical system, alarms, guarding and attachments.
Named appointments
Seven roles, each with a defined authority to stop
The module records who held each appointment for the incident, so the authority to approve the recovery is evidenced rather than assumed.
Incident Commander
Controls emergency response, authority and client notifications and the overall stop-work boundary.
Recovery Supervisor
Sole operational commander for the recovery; may stop the task immediately at any point.
Appointed Lifting / Recovery Engineer
Verifies vehicle condition, centre of gravity, recovery geometry, equipment sizing assumptions, rigging concept and manufacturer-approved connection points.
Geotechnical / Temporary Works Engineer
Confirms ground condition, bearing capacity, edge / slope / void risk, recovery-machine position and any designed ground improvement or load spreading.
Competent Recovery Contractor
Supplies fit-for-purpose recovery equipment, certified personnel and a manufacturer-compatible recovery method.
Plant Engineer / Manufacturer Representative
Determines damage extent and issues written quarantine, repair, inspection and return-to-service criteria.
HSE Manager / Banksman
Establishes exclusion control, communication protocol, rescue contingency, permit checks and the evidence record.
Return to service
Release gates with the evidence each one demands
Plant stays quarantined until every gate is signed by an authorised approver. The signature, role and fingerprint are stored against the incident.
Recovery complete
Recovery plan, permit, supervisor close-out, equipment records and environmental close-out.
Engineering / maintenance release
Written repair, inspection and test release from the plant owner, manufacturer/dealer or competent engineer.
Investigation
Controlled factual evidence, chronology, barrier analysis, RCA and approved CAPA.
Route and ground correction
Revised route, ground, edge and load controls plus any temporary works or platform inspection.
Competence and briefing
Operator and supervisor re-briefing, task MS/RA, pre-use inspection and documented stop-work understanding.
Authority / client / insurer
Required notifications, directions, claims evidence and permit conditions closed or formally controlled.
Notification timing
