Permit to Work at Shift Handover: Where Controls Quietly Disappear
A permit that was safe at 06:00 is not automatically safe at 14:00. How to hand over open permits, isolations and exclusion zones so the controls survive the change of crew.

Most permit systems are designed around issue and close-out. The weak point is the middle: the moment the crew, the supervisor or the permit receiver changes while the work is still open.
At handover the paper may be perfect and the site may not match it. The isolation was reapplied by someone who has gone home, the fire watch was relieved without being told how long to stay, or the exclusion zone was moved to let a delivery through.
What can change between shifts
- The people: a new permit receiver, a new supervisor, a different crew who never saw the method briefing.
- The conditions: heat, wind, light, adjacent work that started after the permit was issued.
- The state of the controls: isolations, barriers, gas readings and exclusion zones that were adjusted during the day.
- The plan: sequence or plant changes agreed verbally and never written back to the permit.
A handover that holds
- Hand over at the workface, not at the cabin. Both people stand where the work is.
- Walk each control on the permit and confirm it physically, not from memory.
- State what changed since issue, even if the answer is nothing.
- The incoming receiver signs for the permit, and the permit conditions are re-read to the new crew.
- If the permit cannot be handed over in person, the work stops and the permit is suspended until it can.
Check your own permit system first. Some authority, client and contractor systems do not allow an open permit to change hands at all: the permit is closed at the end of the shift and a new one is issued. Where that is the rule, the same checks apply to the reissue.
Record what a reviewer will ask for
After an incident or an inspection the first questions are who held the permit at that moment, what they had been told and what they had verified. A handover line with names, time, location and any change answers all three.
Make suspension easy
Handover failures are rarely malicious. They happen when stopping feels expensive. Make it routine: an unconfirmed control suspends the permit, restarting is a short re-check, and nobody is criticised for using it.
