When a client or consultant sends a site observation or non-compliance report, they usually want a plan back quickly. That plan needs clear actions, named owners and real dates. This blank template gives you the structure. Fill it in on your computer and send it back. Everything is blank: no project, company or person details are included.

Tips for a plan that gets accepted
- Name a person for every action, not a department.
- Contain P1 items within 24 hours, and set close-out dates in days or weeks, not months.
- Get to the system failure with the 5 Whys, not just "worker negligence".
- Say what closure evidence you will provide, such as photos, signed checklists or tag registers.
