What's in the method statement form
Ten sections, each with guidance and a pre-filled worked example.
1. Document control
Every controlled MSRA needs a unique reference, revision number and named approver before work starts.
2. Scope of works
Describe exactly what is covered, and state clearly what is NOT covered.
3. Legal and standard references
Cite the authority documents that actually apply to the emirate you are working in.
4. Roles and responsibilities
Name a person for each duty — not just a job title with nobody behind it.
5. Plant, equipment, materials and PPE
List certification and inspection requirements next to each item.
6. Sequence of works (step by step)
One row per step: activity, the hazards created by that step, and the controls that remove or reduce them.
7. Risk assessment register
Score each hazard before controls (initial) and after controls (residual) using the 5x5 matrix. Anything residual 15 or above must not proceed.
8. Emergency arrangements
Cover rescue as well as evacuation — a harness rescue plan is required for work at height.
9. Training, briefing, monitoring and review
Prove the team understood the MSRA — a signature sheet in the language they speak.
10. Approval and briefing sign-off
No work starts until all three approval signatures and the operative register are complete.
How the 5×5 risk scoring works
Score each hazard before controls (initial) and again after controls (residual): severity × likelihood. Anything residual 15 or above must not proceed.
Severity scale
- 1 — Negligible: No injury or minor discomfort, no lost time
- 2 — Minor: First aid case, back to work same shift
- 3 — Moderate: Medical treatment or lost-time injury, reportable
- 4 — Major: Serious injury, permanent disability, major property loss
- 5 — Catastrophic: Fatality or multiple serious injuries
Likelihood scale
- 1 — Rare: Very unlikely, not known to have happened
- 2 — Unlikely: Could happen but not expected
- 3 — Possible: Might happen at some time on this site
- 4 — Likely: Will probably happen during the works
- 5 — Almost certain: Expected to happen, or happens regularly
