Permit-to-Work Audit Checklist (UAE High-Risk Works)

A permit system fails quietly. The forms keep getting signed, the register keeps filling, and nobody notices that the isolation certificate stopped being attached three months ago. This audit checklist is written to be worked through with permits in your hand, on site, not at a desk. Aligned with ADOSH-SF v4.1 permit requirements and common UAE client HSE specifications — not certified by any authority.

System-level checks

• A written PTW procedure exists, is current, and names which activities require which permit type.
• Issuers and receivers are appointed in writing, trained, and the appointment list matches the signatures appearing on live permits.
• A permit register exists and reconciles with the permits physically displayed at the work fronts.
• Permit conflicts (simultaneous operations) are checked before issue — hot work above a confined space entry is the classic UAE finding.
• Permits are time-bound to a shift, not open-ended, and night work is issued separately.

Permit-in-hand checks (sample five live permits)

1. Is the permit displayed at the work front, legible, and within its validity window?
2. Does the description of work match what people are actually doing right now?
3. Are the named receiver and the supervisor present on site?
4. Are the referenced RAMS attached or clearly traceable, and were they briefed to the crew present?
5. Are the specified controls physically in place — gas test record, isolation certificate, fire watch, edge protection, rescue plan, standby person?
6. Have monitoring readings been taken at the stated frequency, with times written down?
7. Is there a signed handback for the last expired permit for the same work front?

Defects most often cited

Closure and record retention

• Closed permits retained with attachments for the retention period in your contract (commonly the project duration plus five years).
• Cancelled and suspended permits retained too — auditors read those first.
• Monthly trend review: permits by type, cancellations, expired-without-closure count, and repeat defects by subcontractor.

Score the audit honestly. A permit system that scores 100% on paper and fails two of five permit-in-hand checks is a paper system, and it will be read that way after an incident.
