Permit to Work (PTW) Safety

1. Purpose of PTW
A formal written authorisation to carry out high-risk work under defined controls. It confirms hazards are identified, controls in place, isolations verified, competent people assigned and communication established.
2. Types of Permit
Hot Work, Confined Space, Excavation, Working at Height (> 1.8 m without edge protection), Electrical Isolation, Lifting Operations, Line Break, Radiography, Roof Access. Each has its own template and validity (max 12 hours).
3. Four-Step Process
1) Preparation — MSRA reviewed, JSA at task location, isolations planned. 2) Issue — Issuer walks the task with Receiver, signs permit. 3) Work — permit displayed at task, changes trigger re-issue. 4) Close-out — Receiver returns permit, area handed back, isolations restored.
4. Issuer & Receiver Roles
Issuer: competent, authorised in writing, independent from the doing team, verifies conditions on site. Receiver: supervisor of the work, briefs the crew, keeps permit visible, stops work if conditions change.
5. MSRA Linkage
The permit references the MSRA and JSA. Controls listed on the permit must match the MSRA. If site conditions differ from the MSRA, the permit is not issued until the MSRA is amended and re-approved.
6. Bypass Consequences
Bypassing PTW is a Category A violation: site removal, disciplinary action, contractor performance strike, and possible enforcement by ADOSH-SF (formerly OSHAD)/Trakhees. Multiple UAE fatalities have been linked to unauthorised hot work and confined space entry.
