1. Governing Principle
Under ADOSH-SF CoP 29.0 an excavation PTW is required where the risk assessment identifies that a permit system is necessary, and it must align with CoP 21.0 (Permit to Work Systems). There is no safe universal depth threshold that removes the need for risk-based controls. A PTW does not replace an approved temporary-works design, excavation MSRA, utility clearance, dewatering design, lifting plan, inspection regime or emergency rescue plan.
2. Temporary Works, Ground & Adjacent Structures
Sheet piles, walings, struts, anchors and the excavated ground act as one system. Loss of strut load, wall movement, toe instability, excess surcharge or unauthorised alteration can cause sudden collapse. The primary excavation PTW must reference the temporary-works design release, latest inspection, monitoring baseline and authorised sequence. Support is never modified or removed under a live permit without reauthorisation.
3. Buried & Overhead Services
Excavation or pile driving may strike, rupture or undermine utilities. Controls: latest drawings, utility-owner NOC, permit-to-dig/service clearance, detection survey, marked exclusion zones, trial pits by safe technique, isolation/support plan, and overhead-line stand-off with plant restrictions. Linkage: excavation/ground-disturbance PTW plus utility NOC and, where required, a verified isolation certificate.
4. Dewatering, Groundwater & Discharge
Drawdown can destabilise soil, induce settlement beyond the sheet piles or cause piping and boiling; pumped water can pollute drainage. Controls: approved dewatering design, duty and standby pumps, back-up power, groundwater and settlement monitoring, protected discharge route, silt control, sampling where required, and pump-failure response. Linkage: excavation or dewatering PTW plus the local-authority dewatering/discharge permit or NOC.
5. Entry, Atmosphere and Confined Excavations
Entry hazards are collapse, falls from the edge and being struck by plant or spoil — sequence support with excavation, provide edge protection, designed access/egress, segregated plant and spoil zones, and competent-person inspection before work and each shift. Deep excavations, sumps, culverts and cofferdams may meet the confined-space definition; CoP 29.0 directs deep and confined excavations to CoP 27.0, requiring a confined-space entry permit and gas-test certificate under the controlling PTW.
6. Driving, Lifting, Height, Hot Work and Traffic
Sheet-pile driving and extraction bring vibration, noise and exclusion-zone control; pile pitching and waling installation are lifting operations needing a lift plan and, where criteria are met, a critical-lift authorisation; work on pile tops or walings needs the site work-at-height permit with rescue provision; welding and burning need a hot-work permit with fire watch; road interfaces need a traffic/road-occupancy permit, with Traffic Police approval where excavation is on roads.
7. Linked Certificates Register
Prerequisite or linked to the primary PTW: utility NOC / service-clearance certificate; isolation certificate (which never authorises work by itself); confined-space entry permit and gas-test certificate; hot-work permit; critical-lift authorisation and lift plan; dewatering and discharge permit/NOC; road/traffic permit. Each must be attached or cross-referenced with its validity period.
8. Workflow, Revalidation and Stop-Work Triggers
Issuer and holder complete a documented pre-job walkdown once design, MSRA, utility clearance, temporary-works release and discharge approvals exist. The PTW states the exact work zone, phase, support configuration and water-control status — a vague 'deep excavation' permit is insufficient. Stop-work triggers: water ingress beyond design allowance; pump failure; excessive settlement or vibration; damage or movement to sheets, walings or struts; unplanned surcharge or plant encroachment; altered service information; adverse atmospheric test; inadequate access or edge protection; loss of emergency-rescue readiness.
