1. The 5 m Position

CoP 29.0 is risk-based: it requires an excavation risk assessment and a PTW system where the risk requires one. It does not create a stand-alone rule that every excavation over 5 m needs a unique emergency permit. A 5 m excavation should still be treated by the project as high-risk deep excavation, because collapse, water ingress, atmospheric hazards, delayed rescue and fall consequences escalate materially with depth.

2. Permit Validity & Handover Duty

Under CoP 21.0 a PTW is limited to one working shift or 12 hours, whichever is shorter. Work continuing beyond that needs revalidation by the permit issuer, based on a site inspection, or a new permit. A formal handover between off-going and on-coming Permit Authority, Permit Holder and work party is mandatory, communicated in a language all relevant personnel understand.

3. Interface Planning Before the Shift

The main contractor coordinates excavation, sheet-piling/temporary-works, dewatering and electrical contractors and follow-on trades, identifying conflicting work, common plant and discharge routes, lifting activities and isolation interfaces. Evidence: master permit register, SIMOPS/interface meeting record and revised MSRA. CoP 21.0 requires a competent person to manage contractor interfaces when multiple PTWs are live.

4. Off-Going Inspection and Face-to-Face Handover

The off-going Permit Holder and competent excavation supervisor inspect the full work face before release: sheet piles, walings, struts and anchors, excavation faces, access/egress, barriers, plant controls, water level, sumps, pump status, generator/fuel and discharge controls. The exchange then covers exact scope, work-front status, live permits, support stage, restricted areas, active isolations, outstanding defects, changes since issue, monitoring values, weather forecast and emergency status; the Authorised Person hands over PTW authority and verifies incoming competency and appointments.

5. Dewatering and Temporary-Works Status

Record groundwater level and trend against action limits; duty and standby pump availability; operating hours; standby-generator status and fuel; discharge route condition; water quality and silt controls; sump condition; leakage, blockage, hose restraint and alarms. For temporary works, record installed support against the approved drawing and sequence, strut/waler condition, pile movement, settlement and vibration readings, spoil and surcharge location, plant exclusion limits and any open design hold point.

6. Minimum Handover Information & Triggers

Permit status — number, expiry, linked certificates, NOCs, discharge approval, live-work restrictions; trigger: permit expired, AP/Holder change incomplete, certificate missing or invalid. Ground and temporary works; trigger: new cracks, deflection, distressed strut or waling, lost support, reading at action limit, unapproved change. Water control; trigger: rising or in-rushing water, pump or generator failure, discharge blockage, uncontrolled silt/oil discharge, drawdown or settlement beyond limits. Emergency readiness; trigger: rescue team or equipment unavailable, communication failure, obstructed emergency access, staff unfamiliar with the plan.

7. Emergency Scenarios

Collapse or loss of support — raise alarm, stop all excavation, piling, lifting and plant movement, evacuate the excavation and exclusion zone, prevent ad-hoc rescue entry, notify the temporary-works engineer and emergency services; re-entry only after competent temporary-works assessment. Flooding or pump failure — evacuate, isolate energy safely, start contingency pumping only when safe. Fall or medical emergency at depth — stop adjacent plant and lifting, protect the casualty, use the planned recovery method, never send untrained rescuers down. Fire or hot-work incident — stop hot work, isolate fuel and energy if safe, alarm, evacuate.

8. Pre-Issue Verification and PTW Statement

No-go criteria before issue: approved, protected and illuminated access/egress capable of casualty recovery; a competent rescue team identified for every shift including nights with defined roles; inspected rescue equipment (communications, first-aid, stretcher/litter, retrieval or lifting system, fall-arrest retrieval, lighting, gas monitor, ventilation, confined-space rescue kit); tested duty and standby pumps with independent power and fuel; completed confined-space assessment with valid gas-test certificate where applicable; and a documented drill or readiness check with a briefed work party. The issuer signs only if the support and dewatering systems are in their authorised condition, inspections, monitoring and dewatering logs are satisfactory, utility, isolation and discharge controls are valid, the site-specific rescue plan is operational, rescue resources are immediately available for that shift, and all work parties are briefed in a language they understand.
