Change classification rules
Minor change
No change to loads, sequence, temporary works, permit conditions, interfaces or the risk profile. Like-for-like substitution only.
Authoriser: Works supervisor + HSE officer
- Record in the MOC register with a reason
- Confirm no permit condition is affected
- Brief the crew before the next shift
Significant change
Changes sequence, plant, access, temporary works detail or introduces a new interface, but stays inside the approved design intent and permit scope.
Authoriser: Project manager + HSE manager + temporary works coordinator
- Designer or competent person re-check before implementation
- Revised MS/RA issued and re-briefed; superseded version withdrawn
- Critical controls re-verified before restart
- Consultant / client notified in writing
Major change
Alters design intent, structural loading, permit conditions, authority approvals, third-party interfaces, or the emergency / evacuation arrangement.
Authoriser: Client / consultant + original designer + authority as applicable
- Stop the affected activity until authorised
- Original designer re-issues stamped design
- Authority re-approval or permit amendment obtained in writing
- Third parties (neighbours, road authority, utility owner) re-notified
- Full re-brief and critical control verification before restart
