# Multi-Case Quarantine and Clinician Return Report — Quality-Control Notes

## Multi-Case Quarantine and Disinfection Protocol

The three-page protocol document was visually reviewed. The document-control table, activation matrix, first-120-minutes response table, authority-directed facility controls, quarters control sequence, decontamination/disinfection stages, medical return-report boundary, RACI matrix, evidence pack and references are readable, aligned and free from visible overflow.

The protocol correctly states that several positive food handlers constitute a Health Authority-controlled cluster event. It does not present the camp as authorised to diagnose, cohort, medically clear or self-release affected workers. It also correctly distinguishes health-facility isolation-room requirements from temporary quarantine outside health facilities under Health Authority supervision.

The decontamination section correctly avoids prescribing one universal disinfectant, dilution or contact time, and instead requires approved products to be used strictly to authority direction and manufacturer label/SDS instructions.

## Clinician-Controlled Return / Continued Restriction Report

The return-report template was visually reviewed. The patient/case identification table, confidential clinical assessment section, minimum-necessary operational status certificate, clinician declaration and camp operational receipt are all legible and professionally formatted.

The template correctly states that it is not the statutory communicable-disease reporting form and must be completed, signed and stamped only by the treating licensed clinician or authorised medical facility. It also preserves confidentiality by separating the clinical section from the employer/camp operational copy.

## Remaining Verification Step

The Excel workbook still requires visual confirmation to verify that the quarantine-placement log, decontamination log, return-release register and daily coordination sheets are clearly structured for controlled site use.

## Multi-Case Quarantine, Decontamination and Return-Control Workbook

The workbook was visually checked and contains seven worksheets: incident dashboard; multi-case medical restriction control; Health Authority/clinician placement log; quarter readiness checklist; decontamination/disinfection control log; clinician-controlled return/release register; and daily multi-case coordination/welfare log. The dashboard uses formula-driven status counts. Status fields apply data validation and conditional formatting to highlight restricted, pending, complete/closed and emergency escalation states.

The workbook retains confidential handling through its instruction banner and uses authority/clinical reference fields rather than diagnosis, laboratory result or specimen details. The quarantine readiness sheet specifically requires an authority direction reference before accommodation/quarters controls are activated.
